TO DO LIST

IMMEDIATELY:

1. ATTEND THE INFORMATIONAL MEETINGS

We will have a final meeting on April 6, 2009 at 7:00 PM in the LSM cafeteria.

2. PAY IN FULL.

3. GET YOUR PASSPORT ASAP

Make an appointment at the Burlington Post Office to submit your application and have your picture taken. For further information, visit the passport link below in the Important Links section. Submit your application before the end of January to ensure that you receive your passport before we leave.

4. MAKE SURE THAT EF HAS YOUR NAME SPELLED CORRECTLY.

BY FEBRUARY 20, 2009:

1. MAKE 3 COPIES OF YOUR PASSPORT. LEAVE ONE AT HOME AND GIVE TWO TO MS. LOVEDAY.

2. COPY BOTH SIDES OF YOUR INSURANCE CARD FOR MS. LOVEDAY.

3. COMPLETE AND RETURN ALL NECESSARY FORMS TO MS. LOVEDAY NO LATER THAN 2/20/09.

4. SUBMIT $150 PAYMENT FOR BUS/TIPS TO MS. LOVEDAY. All checks should be made out to Lewis S. Mills High School.

5. ASK BANK FOR EUROS/PREPAID DEBIT CARDS/ETC.

BEFORE DEPARTURE (APRIL 9, 2009):

1. ATTEND FINAL MEETING ON MONDAY, APRIL 6 AT 7:00 P.M IN THE LSM CAFETERIA..

2. PACK LIGHTLY AND MAKE SURE YOU HAVE ENOUGH BATTERIES FOR YOUR CAMERA.

3. BE IN SCHOOL ON APRIL 9, 2009, NO EXCEPTIONS!

NOTE: Please turn in all forms to Ms. Loveday no later than February 20, 2009. If you begin taking a medication after 2/20/09 that you will need on the France trip, please submit an additional Medication Authorization form to me.
